What is prior authorization?

Prior authorization means your plan requires approval before it will cover certain services, supplies, or medications. Your doctor may need to provide information showing that the service or medication is medically necessary. Medicare Advantage and Part D plans may use prior authorization. If a request is denied, you may have rights to appeal.
